I paid for and got the inspection done, cost me $140 took the pic of the form, uploaded it, waited, and they are telling me it’s the wrong form, with out ever providing a link or clue as to how to get the correct form!

Same thing happened to me with Lyft. If at the top of the sheet it says that it is the inspection certification that means it is the correct form, so try submitting it a few more times. If that doesn’t work contact them and tell them it is the correct form for your state.
